Jordan’s Principle Service Coordinator

- Employment Type: Full Time – 35 hours per week

The Opportunity- Serves as a point of contact for Indigenous children and their families in the CSFS service delivery area, health care providers, and First Nations communities and organizations who are requesting support and advocacy directly related to children and youth.
- Ensures the appropriate and effective sharing of information, including identifying and maintaining contact with Indigenous children and families as required for continuity of care, employing multiple communication strategies including telehealth.
- Collaborates with a multi-disciplinary team to provide a timely response when children are in need of care.
- Develops and documents a coordinated service plan that includes referral, assessment, treatment/admission, discharge (if applicable), transition, and ongoing care protocols.
The Role
- Bachelor’s degree in a health or social services program or related discipline preferred.
- A diploma in a Human Services related field required.
- Three (3) years’ recent related experience working in health and social services, particularly with Indigenous Peoples.

Wage RangeThe target hiring range for this position is $35.07 to $45.43 per hour. Candidates will typically be hired, transferred or promoted between the minimum and midpoint of the wage range, depending on role‑specific knowledge, skills, and experience.
