THIS POSITION IS BASED OUT OF PRINCE ALBERT.

JORDAN’S PRINCIPLE COORDINATOR RESPONSIBILITIES

  • Providing case management services to all children under the Jordan's Principle program.
  • Preparing and submitting proposals to the Jordan's Principle Regional Focal Points for review.
  • Managing funds provided through the program for services/supports approved for families.
  • Monitoring and tracking budgets andhann proposals submitted to the JP program.
  • Identifying children withatham unmet needs and assisting families to secure access to needed services and supports.
  • Building relationships with service providers, and federal or provincial programs and services.
  • Ensuring children receive quality and culturally appropriate health and social services.
  • Networking and connecting families with children with special needs.
  • Arranging transportation and attending appointments with families, if requested.
  • Setting up culturalților and other supports for families.
  • Supporting data collection and analytical activities.
  • Establishing and implementing a communicationייז and support network.
  • Creating awareness of the Jordan's Principle Program in the communities through presentations, pamphlets, etc.
  • Researching and informing staff on best practice models.
  • Compiling monthly and yearly statistical and activity reports as required.
