Care Coordinator - Preston & Burnley

Make a real difference to families and young people in Lancashire

HCRG Care Group is looking for a compassionate, organised and customer‑focused Care Coordinator to join our Care Coordination Hub. This is a fantastic opportunity to be part of a supportive team helping children, young people and families access the right care at the right time.

What you’ll do

As the first point of contact for patients, parents, carers and professionals, you will play a key role in delivering an excellent customer experience. In this role, you will:

  • Provide a friendly, efficient and professional response to all incoming telephone calls and emails into the Care Coordination Hub.
  • Filter, prioritise and direct queries to the correct Lancashire Healthy Young People & Family Services teams.
  • Support clinical teams by booking and scheduling routine clinic appointments in line with service rules and protocols.
  • Ensure referrals and enquiries are signposted promptly to the appropriate services within required timescales.
  • Work collaboratively with colleagues across the service to ensure smooth and effective coordination of care.

What we're looking for

You will thrive in this role if you:

  • Have excellent communication and customer service skills.
  • Can remain calm and supportive when managing a high volume of calls or complex queries.
  • Are highly organised with strong attention to detail.
  • Enjoy working in a busy, fast‑paced team environment.
  • Are confident using IT systems and eager to learn new processes.

Experience in healthcare or coordination roles is desirable but not essential — your people skills and proactive approach matter most.

This role is based between Preston and Burnley so ability and willingness to travel across both hubs is essential.
