• Position: Nurse (RN or LPN – Direct Care)
• Locations: Prescott & Prescott Valley
• Shifts Available: Overnight
• Schedule Options: Full-time, Part-time, PRN
• Setting: Small nursing-supported group homes (4 patients)
• Acuity: Low to moderate
• Work Environment: Stable routines, consistent residents, non violent
• On-Call: As assigned per policy (no constant on-call expectations)

About the Role:

RN and LPN roles function in the same direct-care nursing capacity within our group homes, in accordance with Arizona scope of practice and organizational policy.

You will provide hands-on nursing care in a residential setting where quality of care, safety, and dignity are the priority—not patient volume.

Key Responsibilities:

• Provide direct nursing care in a nursing-supported group home setting
• Administer medications and treatments as prescribed
• Monitor residents' health status and document changes appropriately
• Follow individualized care plans and nursing protocols
• Communicate changes in condition to supervisors and healthcare providers
• Ensure compliance with DDD, ADHS, and organizational standards

Work Environment & Support:

• Small resident population
• On-site support staff present
• Clinical leadership available for consultation
• Clear policies, procedures, and escalation protocols
• Team-oriented environment where nurses are supported—not isolated

Qualifications:

• Current Arizona RN or LPN license in good standing
• Experience with individuals with disabilities or in group home settings preferred (but not required)
• Strong communication and teamwork skills
• Compassionate, patient-centered approach to care
• Ability to pass background and compliance requirements
