Position Overview: We are seeking a passionate and skilled Diabetic Infusion Nurse Practitioner to join our dynamic team. In this role, you will play a pivotal part in providing comprehensive care to diabetic patients requiring infusion therapy. Responsibilities
  • Conduct thorough patient assessments and develop individualized care plans.
  • Administer and monitor diabetic infusion therapy, ensuring patient safety and comfort.
  • Educate patients and their families on diabetes management techniques, lifestyle modifications, and medication adherence.
  • Collaborate with other healthcare professionals to optimize patient outcomes.
  • Maintain accurate medical records and documentation in compliance with regulatory standards.
  • Participate in quality improvement initiatives and contribute to clinic-wide protocols and procedures.
  • Provide compassionate support to patients throughout their treatment journey.
Qualifications
  • Valid Nurse Practitioner license and certification in AZ.
  • Master s or Doctoral degree in Nursing from an accredited program.
  • Minimum of 2 years of experience as a Nurse Practitioner, preferably in diabetes care or infusion therapy.
  • Proficiency in conducting patient assessments, interpreting lab results, and formulating treatment plans.
  • Excellent communication skills, with the ability to effectively educate and engage patients.
  • Strong teamwork ethic and a collaborative approach to patient care.
  • Commitment to maintaining clinical competency through ongoing education and professional development.
  • Experience with electronic health records (EHR) systems (Athena-health preferred).
