Complex conditions like COPD and congestive heart failure (CHF) are incredibly challenging to manage. People who are already struggling with isolation and the burden of these conditions become increasingly frustrated and disengaged, eventually falling through the cracks and landing in the ER. This cycle perpetuates poor health outcomes and excessive medical costs.

WEDNESDAY AND FRIDAY 8-5PM with 1 hour lunch

• Lead the multidisciplinary team to provide world class care to patients with common lung disorders including COPD, asthma and sleep apnea.
• Care for patients in-clinic and via telehealth during normal business hours Monday-Friday.
• Establish care plan for each patient and supervise team based administration of the care plan.
• Explain the benefits of Nuvo Air Medical’s technology and team based model to patients and support regular engagement with our team.
• Collaborate with referring physicians and other members of the care team.

• A passion for innovation, and quality improvement.
• Experience working with Pulmonary patients.
• A willingness to build value in a new and innovative medical practice.
• Outstanding communication skills, including the ability to be personable and supportive of patients and a multidisciplinary team.
• Experience providing culturally competent, patient centered care, including telehealth.
• Ability to provide engaging and effective clinical training and mentorship for clinical and non-clinical teams.
• Strong computer skills & competence with telemedicine tools & EMR documentation.
• Tolerance or appetite for change and some degree of ambiguity that comes with working in a startup environment.
