**** Deliver compassionate, direct patient care in accordance with state regulations
* Implement care plans developed by the RN and document progress accurately
* Monitor patient conditions and communicate observations to the RN and physician
* Collaborate with the care team to ensure seamless, high-quality service
* Participate in professional development and team meetings

* *** Graduate of an accredited practical/vocational nursing program
* Current LPN/LVN license in the State of Arizona
* 3 years of nursing experience preferred (Home Health or Med Surg a plus)
* Current CPR certification
* Valid driver’s license, auto insurance, and reliable transportation
