Poughkeepsie, New York Program Specialist HERO Vet Tap Full Time 35 hours per week/$20.00 per hour.The HERO/Vet-Tap Program Specialist will act as the assistant to the HERO/Vet-Tap Program Managers and be trained in all aspects of the programs. Responsibilities of this position include but are not limited to the following:
General Program Duties:
- Perform intakes, collect required forms, manage hardcopy, and electronic client records.
- Develop individual service plans for each client in conjunction with the Program manager.
- Assist with surveys, process registrations, provide appropriate referrals
- Support:
Provide safe and non-judgmental environment for members to share their concerns and experiences; provide families/caregivers with information on the program - Program development:
Assist in designing appropriate support and socialization activities; in collaboration with the Program Manager, establish, recruit for, and facilitate support groups; recruit volunteers/mentors and clients for program, and other activities as needed - Regular coordination with Program Manager; assist with educational programs and training
- Program Compliance:
Provide services within program guidelines, seek guidance from Director of Veterans;
Programs if unclear.
Advocacy, Support, and I&R:
- Meet with veterans individually and/or with families to provide support and advocacy as needed; discuss findings and impressions with supervisor; present and explain program to individual and/or family; secure required documentation; address concerns and questions; manage expectations
- Provide service in-home or out-of-home on days and times specified, in accordance with agreements made; consult with supervisor about issues involving possible interventions or uncertainty; provide appropriate referrals.
- Collaborate with Program Manager in setting up quarterly recreation/socialization activities for veterans, their families, and other stakeholders (advisory board, staff, etc.)
Other Duties:
- Reporting:
Maintain and keep current individual client records as required; maintain record of client interactions and service delivery. Complete monthly, quarterly, annual program reports with the Program manager. Attend training and/or supervision meetings as required - Contribution:
Actively participate in cooperative efforts of the Veterans Department; participate in activities that contribute to the overall welfare of the agency; undertake other duties as assigned by the Director of Veterans Programs. - Facilitate the referral and hand off process from the HERO and/or Vet-Tap programs to the Vet2
