Key Responsibilities of the Travel Case Manager Nurse:
Coordinate and oversee the patient's continuum of care across various healthcare settings
Conduct comprehensive assessments and develop individualized care plans
Collaborate with physicians, nurses, social workers, and other care team members to ensure seamless care transitions
Monitor patient progress and modify care plans as needed to achieve clinical and financial goals
Provide education and support to patients and families regarding diagnoses, treatment options, and community resources
Facilitate discharge planning, including referrals to home health, rehabilitation, or long-term care facilities
Maintain compliance with hospital policies, regulatory standards, and documentation requirements
Participate in performance improvement initiatives and case reviews

Requirements  of the Travel Case Manager Nurse:
Active New Yoek Registered Nurse (RN) license
NY PRI Certification
Current BLS certification
1-2 Years Acute Care Discharge Planning Experience
