Job Duties and Responsibilities
  • Provides screenings to potential program participants identified through the local county’s Single Point of Access (SPOA) process; helps directly link potential program participants not identified through SPOA with the necessary resources to become SPOA-identified.
  • Educates potential program participants on PEOPLe USA, the agency’s mission & values, and how the Supported Housing program operates.
  • Provides information on OMH, MRT, or HUD Supported Housing guidelines and general housing.
  • Assists accepted program participants with budgeting, including rental costs, utilities, program subsidies, personal finances, and related costs.
  • Explores housing rental options with participants that fit their financial, environmental, and personal needs.
  • Assists participants in completing rental applications and understanding leasing contract components.
  • Assists participants in managing the moving process and furnishing rentals.
  • Supports participants in budgeting to maintain housing and other basic needs; verifies income and budgeted costs to ensure rental subsidies are up-to-date and accurate.
  • Maintains regular meaningful contact with participants (at least one face-to-face and two indirect contacts per month).
  • Educates participants on health & wellness topics, including peer/self-help resources, recovery, wellness dimensions, community resources, trauma & healing, wellness planning, and natural supports.
  • Helps participants identify and overcome barriers to recovery and access to services, ensuring voices are heard and decisions respected.
  • Builds peer-to-peer connections based on lived experience, empathy, and hope for recovery/wellness.
  • Provides peer support, sharing stories and strategies for living well; coaches clients to achieve whole health goals related to the Eight Dimensions of Wellness.
  • Connects people to services and supports through direct bridging/linking.
  • Develops and maintains positive working relationships with other provider agencies and local housing providers (landlords).
  • Documents all meaningful interactions in electronic records software and maintains hard copies in participants’ files.
  • Attends SPOA meetings, HUD Continuum of Care meetings, and related meetings.
  • Supports PEOPLe USA’s mission to instill hope, empowerment, and self-determination in people living with mental illness.
  • Aligns behaviors with trauma-informed care, customer engagement and satisfaction, mutuality & empathy, and a commitment to recovery.
  • Documents time in Paycor and adheres to allocated hours unless approved by the Director.

Supervisory Responsibility

This position has no supervisory responsibilities.

Work Environment

This job operates mostly in community settings, including each program participant’s housing (scattered site). The role also involves a peer-run non-clinical mental health care setting with professional office environments. Standard office equipment is used (computers, phones, photocopiers, filing cabinets, and fax machines).

The physical demands described here are representative of those that must be met to perform the essential functions of the job. The employee frequently talks, stands, walks, and uses hands; may reach with arms.

Position Type and Expected Hours of Work

This is a full-time position, with expected hours of 40 per week.

Travel

Travel is primarily local during the business day; some out-of-area and overnight travel may be expected. Up to 75% travel.

Required

Education and Experience
  • High School degree or equivalency.
  • Personal lived experience with mental illness or substance use.
  • Clean & valid driver’s license.
Preferred Experience
  • Bachelor's degree
  • Bilingual English/Spanish
  • Significant demonstrable experience in health & human services
Additional Eligibility Qualifications
  • New York State Certified Peer Specialist must be obtained within 6 months of hire.
  • Certified Psychiatric Rehabilitation Practitioner (CPRP).
  • All other staff training must be completed in a timely manner.
