Florence Manor - $29.82/hr

Job Description: The LPN assists with day-to-day operations of an OASAS 820 certified residential program. Under the direction of the Clinical Director, the LPN will ensure Residents’ medical needs and goals are met and routine medical tasks as defined below are completed accurately.

Duties:

• Assist staff with day-to-day operations of the residential program.
• Document notable observations and/or significant interactions with residents.
• Conduct rounds and security checks of the residents and facility.
• Conduct routine and random searches of resident’s personal property.
• Administer breathalyzer and drug screen tests.
• Assist and oversee residents with self-administration of medication.
• Report medical and/or medication related concerns to the RN.
• Provide ongoing care coordination and referrals for primary or specialty care, and referrals to community service providers as needed.
• Conduct public health education groups to Residents as directed by the RN.
• Assist with the monitoring of the electronic medication administration record and maintenance of paper records.
• Assist with staff/client training as needed.

Qualifications:

• Valid driver’s license
• Experience with direct care in residential setting desired, but not required.
• Licensed Practical Nurse (LPN) registered through the NYS Education Department.
