Job Summary
This position is responsible for assisting individuals participating in services with activities of daily living by implementing, supporting, and documenting activities that promote recovery, wellness, and self-determination. It also involves supporting community integration and promoting independence. The role includes caring for and maintaining the service location. The essential functions are aligned with the agency's mission, values, and goals.

Description
The responsibilities include developing and implementing recovery plans, assessing and teaching daily living skills, documenting progress, assisting with medication, encouraging therapeutic activities, supporting social relationships, maintaining cleanliness, preparing meals, and ensuring safety and community access for individuals.

Other Functions
Adhere to policies, perform administrative tasks, participate in meetings and training, and maintain safety standards.

Requirements
Qualifications
High School Diploma or GED required;
Bachelor's degree preferred. 1-2 years of experience in Human Services or relevant life experience. Valid driver’s license and certifications such as CPRP/RPRP or Peer Specialist are preferred.

Skills and Abilities
Proficiency in Microsoft Office, strong teamwork and communication skills, ability to multitask, exercise judgment, and work independently. Knowledge of rehabilitation principles and ability to support individuals empathetically.

Physical and Environmental Requirements
Includes sitting, standing, walking, lifting, carrying, pushing, pulling, climbing, balancing, kneeling, crouching, crawling, reaching, feeling, grasping, and visual acuity, with frequency levels specified.
