Job Title: Adult Social Worker – Response Team – Portsmouth

About the Role:

If you are an experienced social worker, join the dynamic Response team, serving as the ‘front door’ for the adults care and support service. This is an exciting opportunity to make a real difference in the community.

Responsibilities:

Triaging initial referrals into the service, offering information and advice, and signposting individuals to alternative services such as the voluntary sector.

Completing Care Act assessments and creating support plans.

Sourcing care packages when required.

Triaging Staffordshire Children Advice and Support Service (SCAS) referrals.

Undertaking safeguarding enquiries.

Supporting community visits as necessary to ensure individuals receive appropriate care and support.

Managing referrals proactively and providing comprehensive support to those in need.

Qualifications &

Experience:

Must have Social Work England registration, be eligible to work in the UK, hold a degree in Social Work, and have at least 2 years of post-qualified experience.
