Overview

Our Inpatient facilities are situated across two sites, Spinnaker Ward based at St Mary's is currently a 16 bedded Rehabilitation Ward, and Summerlee is currently a 40 bedded unit with rehabilitation beds alongside D2A beds. Our vision is providing quality care which uses a person-centred approach, working closely with a wider multi-disciplinary team to achieve exceptional patient care and outcomes. We are looking for a Rehabilitation Assistant to organise and deliver therapy activities as part of the agreed intervention plan for patients, under the supervision of a qualified Therapist.

In this role, you will assist Therapists in carrying out interventions, ensuring the smooth running of the service. You will provide support in management of risk, developing therapeutic relationships, supporting people to live meaningful and fulfilling lives.

Responsibilities
  • Undertake evidence-based Therapy interventions with patients, as agreed with a therapist, to ensure good clinical outcomes. The post holder will be required to work with the patient to gather information, highlighting the need for further assessment, and completing a clinical record of the intervention including the outcome.
  • Actively promote recovery-focussed care with patients, in conjunction with other health professionals, including achievement of set goals.
  • Support patients with their rehabilitation and recovery, respecting them as individuals, supporting them and understanding their right to express their needs.
  • Facilitate and lead patient-focus groups and activities.
  • Use appropriate approved conflict resolution and management of aggression techniques (e.g., PMVA), to de-escalate challenging situations as required for the role.
  • Undertake clinical and environmental risk assessments and document key risk areas, seeking guidance from more senior staff when dealing with cases indicating a high level of risk.
  • To raise incidents appropriately via the reporting system in an accurate, objective, and concise manner.
  • Provide information on improving health and wellbeing and signposting to other agencies.
  • Be aware of own limitations, seek support when the need arises, maintain professional boundaries at all times.
  • Requires the ability to travel between sites. It is preferable that the postholder is a car driver with a vehicle suitable for business use. We are looking for someone who can demonstrate excellent communication skills, using them to engage, persuade or negotiate with clients with complex, at challenging needs to ensure they are provided with the most appropriate care and support.
