The Community Matron role within the Integrated Neighbourhood Team, in conjunction with a wide range of clinical colleagues and specifically, GP's, Practice Nurses and Social Care professionals, will lead and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of repeat admissions to hospital.

Along with using a medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team, attending the INT whiteboard meeting where appropriate, or referring directly to services such as older persons mental health, physiotherapy or occupational therapy.

The instigation of investigations to assist with diagnosis and review of chronic conditions would also be within the clinicians' competencies such as blood tests, ECG and spirometry.

The role will include the provision of strong leadership to the team, including day to day leadership responsibilities, development and creative leadership, responsive leadership, enabling the team to achieve a well led culture.

• Provide patient centred care.
• Monitor, review and evaluate care interventions to ensure optimum quality is maintained.
• Inform clinical manager of any concerns regarding patients, relatives, visitors or staff that may compromise patients care.
• Develop and maintain constructive working relationships and liaise effectively with all members of the multidisciplinary team so that patients' needs are met.
• Communicate effectively with colleagues, patients and carers so that information is shared in order to meet patients' needs in accordance with data sharing protocols.
• Act as a role model in the promotion of person-centred practice, and challenge practice, which is not person-centred, so that a person-centred culture is maintained.
• Keep updated with relevant clinical developments and use knowledge to enhance standards of care.
• Practice, role model and promote safe and effective extended skills in all area of clinical practice.
• Practice, role model and promote safe and effective recording in line with trust policies and professional standards.
• Encourage a culture of patient wellness and coproduction.
• Demonstrate care, compassion, competence, effective communication, courage and commitment with all patients and carers.
• Ensure patients with palliative care needs and their families have those needs met.
