As a key member of the Hospital Team
, your responsibilities will include:

• Hospital Discharge Planning:
  Complete Care Act assessments and arrange safe, timely discharges through the Discharge to Assess (D2A) model.
• Integrated Working:
  Collaborate with health professionals, therapists, housing officers, and others to coordinate discharge planning and aftercare.
• Reablement Focus:
  Promote independence and recovery, aiming to minimise reliance on long-term care and avoid hospital readmission.
• Safeguarding Adults:
  Respond to concerns in accordance with Section 42 of the Care Act and local safeguarding policies.
• Mental Capacity & Advocacy:
  Conduct Mental Capacity Assessments and best interest decisions; ensure access to advocacy when needed.
• Care Reviews:
  Monitor and evaluate care packages for effectiveness, quality, and value for money.
• Documentation:
  Maintain accurate records in line with legal and local authority standards using electronic case management systems.

• Social Work degree or equivalent.
• Registered with Social Work England.
• Prior experience in adult social care, ideally in hospital discharge, intermediate care, or reablement.
• Solid understanding of the Care Act 2014, Mental Capacity Act, and NHS Discharge to Assess processes.

• Confident working in a multidisciplinary team.
• Strong assessment, planning, and time-management abilities.
• Experience with complex decision-making and working under pressure.
• Knowledge of Continuing Health Care (CHC) and NHS-funded care pathways.
• Familiarity with Mosaic or similar case management systems.
