You will play an essential role in a short-stay rehabilitation unit, requiring hospital setting experience. Here, you’ll be part of a dedicated multidisciplinary team, actively participating in board rounds and MDTs, while completing assessments in line with the Care Act and Mental Capacity Act.

You’ll need to balance speed with precision, ensuring high-quality assessments and records. As a member of the Adults Intermediate Care Team, you’ll also benefit from regular supervision and reflective team sessions.
Pay rate to be confirmed.

• Participate as a crucial member of the multidisciplinary team, including board rounds and MDTs.
• Perform detailed assessments in accordance with the Care Act and Mental Capacity Act.
• Deliver high-quality short-term support in a rehabilitation unit, tailored to adults’ needs.
• Maintain precise records and documentation to ensure effective communication within the team.
• Engage in team meetings and reflective sessions to contribute to ongoing service improvement.

• Experience in a hospital setting, particularly in a short-stay rehabilitation unit.
• Experience working within a multidisciplinary team and participating in board rounds and MDTs.
• Knowledge of the Care Act and Mental Capacity Act.
