Support Worker – Young Adult Transition Service
Support workers needed with great experience in Positive Behaviour Support (PBS), trauma‑based care and behaviours that challenge. The service opened summer 2025 to support young adults transitioning to their own assisted living home. We are still building our team and need experienced care professionals to join us.
Responsibilities- Provide person‑centred care and support for young adults with learning disabilities, autism, epilepsy, mental health and other complex needs.
- Implement PBS methods to manage challenging behaviours and promote positive outcomes.
- Build relationships with service users, families and multidisciplinary teams.
- Attend and support service users in their own homes during the recruitment process to understand their needs.
- Contribute to team meetings and share skills and experiences.
- Support individuals to live independently and lead their best life possible.
- Strong experience in mental health, autism and sensory communication.
- Experience in using PBS methods (qualifications welcome).
- Outstanding social skills and ability to build relationships.
- Fun, sociable, confident – able to welcome everyone.
- UK driving licence desirable but not essential.
- We will pay for your DBS application.
