We are looking for a highly skilled and compassionate registered nurse (RN) to deliver home health care across the Portsmouth, NH to York/Ogunquit, ME territory. Desired shift options include Friday, Saturday, Sunday or Saturday, Sunday, Monday. This is a full‑time position requiring a minimum of six months experience as a home‑health RN.

• Annual salary $90,000–$100,000.

• Perform comprehensive patient start‑of‑care assessments, developing and implementing individualized plans of care.
• Complete OASIS assessments accurately and in a timely manner.
• Provide skilled nursing care in accordance with the plan of care and agency policies.
• Maintain continuity of care by communicating with staff, physicians, family members, and community resources.
• Initiate appropriate referrals when necessary.
• Promote patient health through education and rehabilitative measures.
• Assist patients in learning self‑care techniques.
• Other duties as assigned.

• Current RN license specific to the state of assignment.
• At least one year of clinical RN experience (approval from VP of clinical required for less than one year).
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.
