RN - Case Management

8:00 AM - 4:30 PM with weekend rotation, night shift availability required.

We are seeking an experienced Case Management RN to join our team. This role involves providing expert care coordination, discharge planning, and revenue integrity within an acute hospital setting.

• Utilization management and care coordination.
• Effective discharge planning to ensure smooth transitions for patients.
• Collaboration with interdisciplinary teams to enhance patient outcomes.

• Minimum of 2 years experience in a hospital setting is required.
• Active Compact RN License required at submission.
• BLS certification required.
• Experience in acute hospital RN case management and critical care.
