Overview

Attention Registered Nurses:
Brookhaven Hospice is seeking a PRN RN Case Manager to deliver compassionate, high‑quality home-palliative care. If you thrive on change, care for patients and families, and excel in a fast‑paced, interdisciplinary environment, this is the role for you.

Schedule & Location

We need coverage on Monday, Tuesday, Thursday, and Friday – the busiest admission days. The position serves a wide region, including Dover, Rochester, Portsmouth, Exeter, Newington, and Hampton.

Compensation

$41.00 – $52.00 per hour (Average Pay Range). Final compensation is based on experience, qualifications, and responsibilities.

Responsibilities
  • Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
  • Collaborate closely with the Clinical Director to assess eligibility and suitability of clients for hospice services.
  • Serve as a source of comfort and support, ensuring patients’ comfort and quality of life remain paramount.
  • Apply nursing skills to deliver top‑notch care rooted in proven principles.
  • Educate patients and families to empower active participation in personalized care plans.
  • Support call duties as part of the team’s commitment to being there when it matters most.
Qualifications
  • Current unencumbered Registered Nurse license.
  • Minimum 1 year of RN experience in a medical‑surgical/acute‑care setting (hospice experience a plus).
  • Computer‑savvy with typing and EMR proficiency.
  • CPR certification required by state (current).
