Job Summary and Responsibilities

The Registered Nurse (RN) CM promotes patient‑centered care by coordinating care plans for inpatient and observation patients, managing length of stay, ensuring appropriate resource use, and developing safe discharge plans with the multidisciplinary team.

  • Provides case management services for both inpatient and observation patients as assigned.
  • Identifies patients at risk for adverse outcomes during transitions of care.
  • Performs comprehensive assessment of psychosocial, medical, and discharge needs of patients and families, and evaluates resources available.
  • Reassesses clinical condition and considers readmission risk, developing strategies to mitigate it.
  • Coordinates plan of care and drives discharge planning with collaboration among physicians and the care team.
  • Partners with Social Services to ensure post‑acute needs and level of care are appropriate.
  • Assumes responsibility for timely referral to Social Services when psychosocial determinants of health are identified.
  • Involves patient and family in identifying needs and expectations to develop realistic goals.
  • Evaluates progression of care using evidence‑based tools and approved criteria (Inter Qual), escalating issues through the chain of command.
  • Makes appropriate referrals to third‑party payers and disease‑ and case‑management programs for chronic conditions.
  • Facilitates patient throughput with focus on effective care transition, quality, and efficiency.
  • Documents professional recommendations, discharge plan, care coordination interventions, and case‑management activities.
  • Aligns patient needs with available resources to ensure safe discharge.
  • Acts as a liaison, communicating professionally between physicians, patients/families, hospital staff, and outside agencies.
  • Actively seeks ways to control costs without compromising patient safety, quality of care, or services delivered.
  • Directs activities to identify and provide for under‑resourced patient needs through education programs, assistance activities, and community resources.
  • Participates in performance‑improvement activities, identifying, documenting, and intervening when avoidable days occur.
  • Adheres to established policy and procedure and standards of care; escalates issues promptly.
  • Serves as an advocate for patient’s rights, needs, and values, ensuring cultural and religious considerations are respected.
