Join Our Team as a Clinical Coordinator RN
Are you a compassionate, organized, and motivated RN leader looking to make a real difference in the lives of older adults? We are seeking a dynamic Clinical Coordinator to join our clinical management team.
In this role, you’ll coordinate high-quality resident care, support nursing staff, and help lead our community’s clinical operations with compassion and professionalism. This position works closely with our Resident Care Manager and Director of Nursing to ensure an environment of clinical excellence, regulatory compliance, and resident-centered care—while fostering a positive, supportive culture for both staff and residents.
What You’ll Do:
Leadership & Team Support
Maintain strong floor presence and support staff/resident needs.
Participate in an every-third-week on-call rotation (evenings, nights, weekends).
Assist DON with hiring, performance management, and annual reviews.
Support daily clinical operations and serve as backup for DON/RCM
2. Staffing & Scheduling Management
Develop and manage schedules for LNAs, MNAs, and Charge Nurses.
Ensure adequate staffing levels to meet resident acuity and regulatory requirements.
3. Clinical Compliance & Quality AssuranceEnsure compliance with NH He-P 805 regulations and internal policies.
Assist with state survey readiness, audits, and corrective action planning.
Participate in resident admissions, transfers, and quality initiatives.
Maintain accurate EMS packets, emergency contacts, and clinical documentation
4. Staff Training & DevelopmentDevelop and maintain an annual training plan aligned with NH 805 requirements.
Oversee new hire orientation, including classroom training, skills validation, shadowing, and documentation
Monitor and evaluate staff competency, providing ongoing coaching and feedback.
Maintain and audit service plans, assessments, and documentation.
Requirements for this role consist of:Education & Experience
1–3 years’ experience in assisted living, long-term care, or related healthcare setting.
Experience with NH He-P 805 regulations preferred.
Point Click Care experience strongly preferred.
Experience with staff training, scheduling, or clinical coordination preferred.
LicensureCurrent Registered Nurse (RN) license (state or compact).
Associate or Bachelor s degree in Nursing is required
