Position: Part-Time Licensed Practical Nurse (LPN)

Overview

This position will work as a member of a multi-disciplinary team to deliver comprehensive, person-centered, integrated services to the community with a focus on medication tracking, ordering, handling and auditing. The LPN plays a critical role in medication management and patient care by supporting providers and clients and are also integral members of the treatment team. LPNs work with client access specialists and non-clinical staff to ensure the workflows are efficient, the client experience is positive and medication safety and accuracy is prioritized.

This role includes interaction with clients during medication pass times, offers general assistance with medication self-administration, and supports medical issues or concerns as a key member of the integrated treatment team. This position has access to mental health billing and service records.

Saturday and Sunday, 8:00am – 5:00pm, (Monday - Friday off!)

Compensation $31.97-$42.42 per hour based on years exceeding the Minimum Qualifications

Union AFSCME — This role is a Union Represented position through the American Federation of State, County, and Municipal Employees (AFSCME).

Minimum Qualifications

  • Completion of an accredited course of training for licensed practical nurses.
  • Current licensure with the Oregon State Board of Nursing as a Licensed Practical Nurse.
  • Must meet CCC privileging requirements as required by FTCA.
  • Current BLS CPR certification prior to start date.
  • Ability to adhere to Central City Concern’s drug-free workplace policies and to comply with the Drug-Free Work Place Act of 1988.
  • Must have the ability to read labels and distinguish between colors.
  • Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
  • Must pass a pre-employment drug screen, TB test, and background check (including DHS Background Check Unit clearance).
  • Driving preferred. If driving, must possess a current driver’s license, access to a vehicle, and meet CCC Fleet Safety standards; must pass driver training and maintain minimum auto liability coverage.

Essential Duties And Responsibilities
  • Work as a member of the 16th and Burnside multi-disciplinary team to deliver comprehensive, client-centered, and trauma-informed integrated services.
  • Actively participate in the integration of services and improve collaboration while following information-sharing guidelines.
  • Proactively identify opportunities for workflow improvement and participate in review of Key Performance Indicators and continuous improvement plans.
  • Support diversity and culturally responsive services for clients and families.
  • Track, audit, handle and order medications within the LPN scope.
  • Take primary accountability and coordinate the tracking, auditing and ordering of medications upon admission and during a client’s stay in the facility.
  • Work under the direction of a Licensed Medical Provider (LMP) and RN regarding medication orders and treatments and/or plan of care.
  • Evaluate response to focused plan of care interventions as directed by RN/LMP.
