Summary

The Resident Care Manager (RCM) is a nurse who coordinates all nursing care on an assigned unit and is accountable for managing the nursing care of assigned residents.

Essential Functions

  • Supervises CNAs and Nurses who provide direct care to the assigned residents. Initiates, facilitates, attends, and documents unit meetings and ensures follow-up of identified issues.
  • Collaborates with the interdisciplinary care team to provide comprehensive care management for residents.
  • Acts as a liaison with other staff, residents, and families to ensure the Robison Jewish Home goal of providing care in an atmosphere of comfort, independence, and dignity.
  • Performs quarterly nursing assessments, evaluates progress toward goals, and coordinates interdisciplinary changes in goals and approaches of the care plan. Chairs regularly scheduled care conference interdisciplinary meetings attended by the resident, the resident's legal representative, and anyone designated by the resident.
  • Completes standard nursing assessment tools upon admission. Participates in daily 24-hour report/stand-up process to report on MDS status and MDS change in condition determination with IDT team members.
  • Ensures MDS schedule, PPS, and OBRA assessments are followed and completed in a timely manner. Coordinates with the Health Information Coordinator on MDS schedule/timelines.
  • Provides direct resident care when needed.
Required Skills/Abilities

Consistent execution of quality customer service. Maintains and demonstrates ethical conduct. Able to effectively communicate with geriatric and/or confused residents and a variety of other people such as family members, visitors, vendors, Private Duty Residents, and staff. Positive and constructive attitude.

Supervisory Responsibilities

Assists the Director of Nursing with supervision of direct care staff as part of the Nursing Leadership Team.

Working Environment

This position regularly works on campus. Works in a well-lit and well-ventilated facility. May be exposed to confused, belligerent or physically aggressive residents; may be exposed to contagious diseases and mental stress. Subject to frequent interruptions. Manual dexterity, good vision and hearing are required. Must be able to read product labels and distinguish colors.

Physical Demands

This position involves frequent physical activity, including lifting up to 50 pounds, carrying items short distances, and repeated bending, twisting, and reaching. Employees spend most of the shift walking and standing, with limited time sitting, making the role physically demanding and requiring consistent strength and mobility.

Required Education, Experience, Licenses, and Certifications
  • Graduate of an accredited School of Nursing. Must complete annual continuing education required pursuant to the Oregon Administrative Rules for Oregon Nursing.
  • 2 or more years’ current nursing experience required. Long-term care experience in a supervisory or management position is preferred.
  • Prior experience with medical recordkeeping (Point Click Care or similar) is preferred.
  • Current, unencumbered LPN or RN license with the state of Oregon required. Must be in good standing with the Oregon State Board of Nursing.
  • Current CPR/AED and First Aid certifications required.
