Job OverviewResponsible for quality of care for residents, ensuring individualized needs are met through the nursing process and in compliance with the Nurse Practice Act. Manages nursing care for assigned residents, coordinates with physicians and Interdisciplinary Team members, develops and documents care plans, and supports professional development.
Key Responsibilities- Accountable for quality of care for residents according to individualized needs.
- Manages nursing care of assigned residents, ensuring maximum independence and self‑direction.
- Coordinates care provision with physicians and other interdisciplinary team members in a collegial relationship.
- Ensures the resident's care plan is developed, implemented, and documented to meet individual needs.
- Accepts professional development as an integral part of nursing practice.
- Bachelor's Degree in Nursing.
- Oregon Registered Nurse License upon hire.
- 3 years of nursing experience, including 2 years in care of medically fragile children and/or adults.
- Compliance with Department of Human Services (DHS) background check requirement (every 2 years).
- Recent relevant RN Care Manager or case management experience in a long‑term care environment.
- Master's Degree in Nursing.
- 2 or more years of RN care/case management experience in a long‑term care environment.
- Leadership experience.
