RN Care Manager (RCM) – Long Term Care, Child Center at Providence Portland Medical Center.

This full-time day shift position requires at least 2 years RN Care Manager experience in an adult long-term care facility.

Accountable for quality of care for residents following the nursing process and Nurse Practice Act. Manage nursing care of assigned residents ensuring maximum independence and self‑direction. Coordinate care provision with physicians and interdisciplinary team members. Develop, implement, and document individual care plans meeting residents’ needs. Accept professional development as integral to nursing practice.

• Bachelor’s Degree in Nursing.
• Oregon Registered Nurse License upon hire.
• Criminal background check required every 2 years.
• 3 years nursing experience, 2 years caring for medically fragile children and/or adults.
• Recent relevant experience.

• Master’s Degree in Nursing.
• 2+ years RN Care/case management experience in long‑term care.
• Leadership experience.
