Job Summary

RN Coordinator role consists of the planning and coordination of new referrals, phone triage of existing patients, and the majority of responsibilities are carried out onsite in office. The role may provide direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living site. These services are provided through collaboration and coordination with various providers, the patient and/or caregiver, the physician, and the payor in Oregon and/or southwest Washington.

Schedule: Full Time / Days

• Plan and coordinate new referrals
• Provide phone triage for existing patients
• Deliver direct nursing services and case management for infusion therapy
• Collaborate with providers, patients, caregivers, physicians, and payors

• Oregon Registered Nurse License upon hire
• National Provider BLS - American Heart Association within 30 days of hire
• License in all states where care is delivered
• At least 3 years of Home Infusion nursing experience

• Bachelor's Degree in Nursing
