Registered Nurse (RN) – Triage
Location: Wellness / Indian Health Clinic
Schedule: Full-Time, Temp-to-Perm
Pay Range: $38/hr – $58/hr (DOE)
Department: Medical
Reports To: Nurse Manager
FLSA Status: Non-Exempt
Position SummaryThe Registered Nurse (RN) provides triage services, direct patient care, nursing assessments, and patient education in an outpatient primary care setting. This role supports clinic flow, prenatal and Coumadin case management, HIV data tracking, and ensures culturally appropriate care for all patients. The RN works closely with medical providers and administrative staff to coordinate care and maintain efficient clinic operations.
Essential Duties & Responsibilities Triage & Patient Care- Provide triage care for the clinic, including telephone triage
, walk‑in assessments, and appointment consultations. - Conduct comprehensive nursing assessments and develop appropriate plans of care.
- Deliver direct patient care as needed, ensuring safe and evidence‑based nursing practice.
- Provide health‑related teaching and education tailored to individual patient needs.
- Communicate clearly with patients, families, and providers regarding symptoms, follow‑up plans, and instructions.
- Coumadin Case Management:
- Monitor the Coumadin registry, including tracking, data entry, follow‑ups, and coordinating with providers.
- Ensure timely INR follow‑ups and accurate documentation.
- Prenatal Case Management:
- Maintain the prenatal registry with accurate tracking, data entry, and communication with expecting mothers.
- Support prenatal assessments and follow‑up plans.
- Maintain and update HIV database information following program requirements.
- Chart and maintain medical records in compliance with regulatory and internal documentation standards.
- Assist medical providers by communicating patient status, concerns, and medication needs.
- Participate in ordering and maintaining clinic supplies, medical equipment, and pharmaceuticals.
- Help develop and implement Quality Assurance (QA) procedures.
- Prepare reports accurately and on time as required by leadership or program guidelines.
- Participate in meetings with tribal, local, state, and county partners.
- Ensure all services provided are culturally appropriate and trauma‑informed.
- Collaborate with clinic staff, maintain appropriate professional boundaries, and uphold respectful communication at all times.
- Provide flexible coverage and additional services based on clinic needs, staffing, and skill set.
- Current Registered Nurse (RN) license in Oregon
. - Bachelor of Science in Nursing (BSN) required.
- Minimum 2 years of nursing experience
, preferably in ambulatory care with triage exposure. - Experience using standard nursing tools such as stethoscopes, sphygmomanometers, point‑of‑care testing equipment, scales, etc.
- Ability to adjust care to the unique needs of each patient.
- Strong independent decision‑making skills with the ability to make accurate, evidence‑based clinical judgments quickly.
- Ability to work with minimal supervision while maintaining high standards of care.
- Ability to communicate effectively and confidentially with patients, staff, and external partners.
- Must be able to lift 10–25 lbs as required.
- Must maintain professional boundaries at all times.
- Must pass a pre‑employment drug test, background check, and DMV record screening.
- Prior Diabetic Foot Care Training required.
- Experience working with Native American / Alaska Native communities

- Physical activities include sitting, standing, walking, stooping, bending, lifting, pulling, carrying, and reaching.
- Frequent use of computers and clinical equipment requiring fine motor skills and typing.
- Requires concentrated mental and visual attention to perform complex clinical tasks, documentation, and case management.
- Work performed in an outpatient primary care clinic with generally good working conditions.
- May experience occasional exposure to heat, cold, water, or environmental factors typical of clinical environments.
