Job Description
Park Nicollet is looking to hire a Nurse Navigator to join our team! Come join us as a Partner for Good and help us make an impact on the care and experience that our壬 patients and their families receive every day.
In a collaborative and multidisciplinary environment, the Nurse Navigator coordinates and manages care for suspected or newly diagnosed cancer patients. The role demonstrates a caring, competent manner to instill patient confidence and contributes to an environment that is centered on the needs of patients and families. It is vital to ensure coordination of care through a multidisciplinary approach and collaboration with other Nurse Navigators throughout the cancer care program, empowering physicians and patients in decision support related to cancer and advanced cancer diagnoses.

MINIMUM QUALIFICATIONS Education, Experience or Equivalent CombinationAssociate degree in nursing with 5 years’ experience in oncology care is required.
Experience with or willingness to become educated about specific cancer diagnosis related to navigation.
Licensure/ Registration/ Certification- Oncology Certified Nurse (OCN) or willingness to obtain within 1 to 2 years.
- License as a registered nurse by the State of Minnesota.
- Licensed or ability to receive licensure in Wisconsin.
- Proven proficiency in the operation of the following:
Personal Computer, Telephone, Faxırken, Photocopier, Printer, and electronic medical record.
BSN with 2‑3 years of experience in oncology care preferred.
