Job SummaryA Registered Nurse (RN) in our Skilled Nursing Facility is responsible for delivering high‑quality, person‑centered care to residents, coordinating with interdisciplinary teams, and ensuring compliance with all regulatory standards. This role involves assessing resident needs, implementing individualized care plans, administering medications and treatments, and providing leadership to nursing staff.
Responsibilities- Conduct comprehensive assessments of residents upon admission and as needed.
- Participate in care plan meetings with residents, families, and the care team.
- Provide direct nursing care, including administering medications, treatments, wound care, etc.
- Monitor and document residents’ health status, including vital signs.
- Assist with activities of daily living (ADLs) as needed.
- Ensure care is consistent with residents’ plans and nursing standards.
- Prepare, administer, and document prescribed medications accurately and safely.
- Monitor residents for adverse reactions and report concerns.
- Maintain accurate medication administration records (MARs).
- Serve as a liaison between residents, families, physicians, and other professionals.
- Report significant changes promptly.
- Participate in shift change reports, staff meetings, and interdisciplinary meetings.
- Supervise and provide guidance to LPNs, CNAs, CMAs, and other support staff.
- Assist in orientation and training of new staff.
- Delegate tasks appropriately and oversee staff performance.
- Maintain accurate, timely, thorough documentation in EHR or paper charts.
- Ensure compliance with federal, state, and local regulations.
- Assist in preparing the facility for regulatory inspections and audits.
- Participate in QAPI programs for continuous improvement.
- Follow infection control protocols.
- Report and document incidents, accidents, or safety concerns.
- Advocate for the rights and preferences of residents.
- Perform other duties as assigned.
- Registered Nurse (RN) licensure in the state of practice.
- Previous experience in a skilled nursing facility, long‑term or acute care setting preferred.
- Strong clinical skills and knowledge of geriatric care.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work effectively as part of an interdisciplinary team.
- Proficiency in EHR and basic computer skills.
- Critical thinking and problem‑solving abilities.
- Physically capable of tasks such as lifting, bending, standing, and assisting mobility.
- Ability to stand and walk for extended periods.
