RN Disease Specific Program Coordinator – Legacy Health
Join Legacy Health as an RN Disease Specific Program Coordinator to champion optimal patient care for a disease‑specific population at a site‑based level and to partner with peers across Legacy Health to weave best practices throughout the organization.
Responsibilities- Collaborate with physician and administrative leadership to promote best practices at the site level.
- Serve as a clinical expert for staff providing disease‑specific patient care.
- Facilitate team coordination in the delivery of evidence‑based care.
- Develop and maintain disease‑specific protocols and processes in collaboration with Service Line leadership.
- Support patient and community education.
- Prioritize work, manage time and projects independently with minimal supervision.
Education
Academic degree in nursing (BSN or MSN/MN) required. Internal candidates may meet the degree requirement if a bachelor’s or master’s degree is in progress with an expected completion date within 2 years of the promotion date; progress will be reported periodically to the site Chief Nursing Officer (CNO).
Experience
Two years of experience in the provision and coordination of care for the disease‑specific patient population required, 3 years preferred. Experience leading performance improvement efforts and disease‑specific program management strongly preferred.
Skills
Demonstrated skills in patient assessment, multidisciplinary collaboration, problem solving, data analysis, leading change, teaching, coaching, written and verbal communication. Proficiency in Microsoft Office Suite and clinical information systems.
Licensure
- Current applicable state RN license required.
- For RNs working in both Oregon and Washington, Oregon and Washington RN licensure required.
- BLS through AHA required.
- Specialty certification for the disease‑specific patient population preferred.
USD $57.63 - $86.08 per hour.
