    Position Summary

    The Director of Peer Services (Director) is a leadership position responsible for oversight of all peer mentor services, high quality peer service delivery, expanding peer service resources through all levels of substance use care at Fora, and building and maintaining community partnerships. The role is designed to develop and foster seamless patient access to internal levels of care and to needed community resources.

    This position is vital in coordinating with all levels of care within the Fora Health treatment continuum, ranging from transitional recovery housing, to withdrawal management, residential, outpatient to Fora Health alumni. This position ensures patients needs are identified and met at all points of access and that patients are provided opportunity to access community resources ranging from entitlement benefits, to housing, to employment opportunities.

    The Director of Peer Services champions a positive spirit of recovery and healing within all stages of change and represents Fora within the recovery and behavioral health community. The Director partners with the leadership team to envision and develop expansion of peer services within all levels of treatment. The position cultivates a patient experience that is based on community, connection, and hope.

    Job Functions
    • Develops and maintains programming for peer service delivery philosophy and procedures for Fora recovery housing through project management.
    • Develops and implements Continuous Quality Improvement (CQI) plans for peer mentor services within all levels of care.
    • Evaluates the effectiveness of Peer Mentor Services programs and makes improvements based on key performance indicators.
    • Develops and maintains formalized pathways to community resources for Social Determinates of Health (SDOH) to include employment, housing, benefits, and healthcare.
    • Develops and implements training and interventions for the peer mentor team to support patients stages of change; ranging from harm reduction to abstinence based programming.
    • Partners with Sr. Manager of Development to identify and pursue peer mentor expansion funding. Responsible for contract oversight and management.
    • Leads peer mentor services operations for all levels of care, and coordinates with Peer Mentor Supervisor and Residential Leadership Team to deliver a high‑quality residential treatment program that ensures peer mentor support for housing, physical health, and recovery resources.
    • Coordinates pro‑social recovery activities for patients within all levels of care, which include both internal and external recovery events.
    • Provides a safe, stable and nurturing environment for patients so they may together create a community of healing from addiction while developing the skills and resources to maintain recovery.
    • Maintains collaborative, consultative relationships with clinical and medical leadership for coordination of patient services.
    • Collaborates with internal departments in order to provide seamless patient transitions through the continuum of care.
