They evaluate psychosocial and mental health issues to ensure transition planning, crisis management, and coordination of internal and external services. They assess intervention strategies for behavioral and addiction issues, as well as end‑of‑life care and informed decision‑making. Our Clinical Social Workers advocate not only for patients but also for surrogates, family members, caregivers, and health care providers.

Qualifications

Clinical Social Worker I (CSWA) – Working toward a Licensed Clinical Social Worker license; salary $39.27‑$56.16/hr.

Clinical Social Worker II (LCSW) – Licensed Clinical Social Worker license; salary $41.24‑$58.96/hr.

Responsibilities

Provides social work services in adult and pediatric settings across the health‑care continuum with a focus on holistic patient‑centered care and attention to vulnerable populations. Works independently and collaboratively as part of an interdisciplinary team.

Key services include comprehensive evaluation of psychosocial and mental/behavioral health status, legal and safety considerations, substance use or addiction, chronic illness, or end‑of‑life issues with brief counseling, transition planning, crisis management, and coordination with internal and community stakeholders. Interventions may be with individuals, family members, or groups.

Throughout the care continuum, social workers utilize a strengths‑based approach to enhance the capacity of people to address their own needs.

Education
  • Master of Social Work degree from an accredited School of Social Work required.
Experience
  • Clinical Social Worker I:
    Preferred 1 year of relevant experience in care management, mental health, addictions services, medical social work, or social service organization.
  • Clinical Social Worker II: 2 years relevant experience required; preferred 3 years.
Skills
  • Broad knowledge of social work principles and ability to apply them with individuals, families, and groups, integrating developmental and aging considerations, illness changes, and cultural literacy implications.
  • Knowledge of medical, mental health, substance use diagnoses, disabilities, treatments, medications, and terminology.
  • Strong organizational, communication, and collaboration skills, including triage of patient and family needs and documentation.
  • Knowledge of social determinants of health, diversity/equity/inclusion principles, and culturally responsive practice with historically marginalized populations.
  • Ability to locate, utilize, and advocate for patient/family access to hospital and community resources.
  • Knowledge of legal and regulatory issues and ability to serve as a resource to the team.
  • Competence in navigating electronic medical records and basic computer programs.
  • Knowledge of compliance requirements and current health‑care environment.
Licensure
  • Clinical Social Worker I:
    Clinical Social Work Associate (CSWA) required in Oregon, working toward LCSW; CPR certification (AHA BLS) required.
  • Clinical Social Worker II:
    Licensed Clinical Social Worker (LCSW) required in Oregon; CPR certification (AHA BLS) required.
Pay Range

USD $41.24 – $58.96 per hour.
