Position: Dual Diagnosis Counselor- Out Patient

Dual Diagnosis Counselor – Outpatient

Fora Health Treatment & Recovery is seeking a dedicated Dual Diagnosis Counselor to provide clinical services to adult outpatient clients. The counselor will work under the direction of the Outpatient Manager and collaborate with a multidisciplinary team to support clients in recovering from the effects of substance use and mental health disorders.

Essential Job Functions
  • Conduct thorough clinical evaluations, develop collaborative treatment plans, and deliver individual and group therapy.
  • Utilize the DSM‑5 and ASAM criteria to assess and diagnose substance use and mental health disorders upon enrollment.
  • Document treatment progress, competencies, and outcomes; apply placement, continued stay, and discharge criteria.
  • Perform continuing care, relapse prevention, and discharge planning in collaboration with significant others.
  • Coordinate referrals and communicate with referral agencies regarding screenings and treatment planning.
  • Maintain client confidentiality and navigate regulatory exceptions with clarity.
  • Collaborate with all service areas and agency staff to provide appropriate client referrals.
Counseling and Education
  • Establish a warm, respectful, and empathetic therapeutic relationship.
  • Facilitate client engagement in the treatment and recovery process; role model effective milieu therapy interventions.
  • Provide individual and group counseling to achieve treatment goals; support family therapy as needed.
  • Teach life skills including stress management, communication, and refusal skills.
  • Address treatment‑interfering behaviors with appropriate therapeutic interventions.
Referral
  • Arrange or assist clients in following through with referrals to other professionals and community programs.
  • Explain referral processes and facilitate transitions to continuing care resources.
Professional and Ethical Responsibilities
  • Adhere to professional codes of ethics and safeguard client standards.
  • Comply with federal and state laws and agency policies regarding substance use disorder treatment.
  • Participate in ongoing supervision and consultation; maintain current addiction counselor certification and QMHP privileging.
  • Maintain healthy boundaries with clients and families.
Team Participation
  • Attend treatment team and clinical supervision meetings.
  • Maintain strong working relationships with team members across departments.
Additional

Job Responsibilities
  • Complete additional trainings as assigned in a timely manner.
  • Participate in special projects and other duties as assigned.
Supervisory Functions
  • This position has no supervisory duties.
Qualifications
  • Graduate degree in psychology, social work, or related behavioral health field.
  • QMHP credential per OARs; ability to conduct comprehensive assessments and write DSM diagnoses.
  • CADC I credential or ability to qualify within 6 months; CADC II/III preferred.
  • Knowledge of substance use disorders, basic psychopharmacology, and their impact on mental health.
  • Skilled in motivational interviewing, DBT, and other evidence‑based practices.
  • Effective communication, rapport building, and crisis de‑escalation skills.
  • Ability to maintain clinical documentation and facilitate groups.
Other Requirements
  • Minimum two years of sobriety for employees with a history of substance use disorder.
  • Pass pre‑employment or for‑cause drug test and DHS criminal background check.
  • Regular cell phone connectivity for work communication.
  • Registration with Oregon Health Authority (DMAP) and National Provider Identifier (NPI).
Working Conditions
  • Indoor office environment with infrequent travel; occasional evening/ weekend work.
  • Exposure to communicable diseases (TB, Hepatitis, HIV, etc.) and potential traumatic environments.
  • Potential use of cleaning chemicals; PPE provided.
  • Shift work may include weekdays, weekends, and holidays.
