Minimum Qualifications
  • Must be licensed as an Occupational Therapist in the state of Oregon
  • Graduate from an accredited program with a degree in Occupational Therapy
  • CPR Certification
  • National Provider Identifier (NPI) required upon hire
Preferred Qualifications
  • Experience in Home Health
  • Organizational skills to facilitate program development
Responsibilities
  • Establish treatment plan appropriate to physician orders
  • Prepare/complete clinical and progress notes
  • Teach patient and family/caregivers in meeting individual treatment goals
