MDS Coordinator/Resident Care Manager - RN

Schedule:
Monday thru Friday 9am-5pm (on call rotation for weekends and weekdays)

Salary: $105k - $115k / year DOE

Mirabella Portland, a part of Pacific Retirement Services, is a beautiful not-for-profit Continuing Care Retirement Community (CCRC) located in the SW Waterfront. We are hiring a full time MDS/Resident Care Manager for our 27 bed health center (Skilled nursing/Long term care).

What You’ll Be Doing

As the MDS Coordinator & Care Manager, you will ensure the delivery of holistic, person-centered, individualized, and clinically competent care. You’ll coordinate and collaborate with the Interdisciplinary Team (IDT) to oversee the care of both short-term and long-term residents. This role ensures that services are appropriate, continuously evaluated, and designed to help residents achieve their highest practicable level of health, function, and quality of life.

  • Lead, coach, and support nursing team members to ensure accountability, communication, and clinical excellence.
Resident Assessment & Care Coordination
  • Maintain compliance with all state and federal RAI/MDS regulations.
  • Complete and coordinate accurate and timely documentation of the Minimum Data Set (MDS) per PDPM and OBRA schedules.
  • Collaborate with therapy and the IDT to determine the Assessment Reference Date (ARD) and ensure supporting documentation is in place.
  • Develop, review, and update individualized Care Plans based on Care Area Assessments (CAAs), resident interviews, and changing conditions.
  • Facilitate and participate in admissions, weekly, and quarterly assessments and care conferences.
  • Collaborate with IDT for discharge planning and transitions of care, including communication with managed care providers.
Clinical Oversight & Quality Assurance
  • Coordinate and direct nursing functions and ensure physician orders are carried out appropriately.
  • Meet daily with charge nurses to review changes in resident conditions, delegate tasks, and ensure follow‑up.
  • Review and investigate incident reports, document findings, and provide follow‑up to the Director of Nursing or Health Services Administrator.
  • Review and audit ADL documentation to ensure accurate representation of resident function; identify and address staff training needs.
  • Participate in Medicare/Insurance utilization reviews, Triple Check billing reviews, and QAPI/PIP initiatives.
  • Collaborate with the Medical Records Coordinator on ICD‑10 coding and data accuracy.
  • Follow up on Pharmacy Consultant Reports and ensure medication and treatment processes align with regulations and facility policies.
Collaboration & Communication
  • Establish open, ongoing communication with residents, families, and healthcare representatives.
  • Work closely with therapy, restorative nursing, and interdisciplinary partners to promote residents’ optimal levels of function and well‑being.
  • Assist with staff onboarding, training, and competency development in partnership with the Director of Staff Development.
Additional Duties
  • Maintain and communicate the MDS calendar to the IDT.
  • Transmit MDS data and ensure timely submission.
  • Participate in High Risk Area (HRA) reviews, including psychotropics and SAR/NAR assessments.
  • Take rotating on‑call responsibilities or provide nursing coverage as needed.
  • Perform other duties as assigned.
Qualifications
  • Current RN license in Oregon.
  • Experience with MDS coordination, care planning, and clinical leadership in a skilled nursing or long‑term care setting.
  • Strong communication, organizational, and problem‑solving skills.
  • Working knowledge of PDPM, OBRA, RAI processes, and CMS regulations.
