Purpose:
member of Rehabilitation services patient care team providing occupational therapy services such as ADL training, energy conservation, splint fabrication upper extremities, exercise, sensory integrations, perceptual/cognitive training to assist patients in improving function at home and in the community. Treatment includes patient/family education and therapeutic procedures to improve function, strength, endurance and functional mobility. Therapists evaluate each referred patient, establish treatment goals, develop and implement treatment plan, assist in discharge planning and post-hospital placement.
Therapists are expected to supervise therapy interns, assist with educational research activities and meet hospital documentation and productivity standards.
Shift flex: may include weekend shift rotation. Relief employees must be available to work at least four (4) days per pay period as determined by department needs.
Function/Duties of PositionEvaluation and Caseload Management
- Prioritize patient care needs given medical necessity, discharge plan, medical stability and appropriateness to participate in rehabilitation.
- Evaluate patients to determine appropriateness of referral.
- Identify rehab potential, establish treatment goals specific to scope of acute care rehabilitation, including functional outcomes, quantitative measures and target dates.
- Develop treatment plans to achieve identified goals.
- Document evaluations in patient medical record in compliance with DNV, OHSU, OAR’s and third‑party payer criteria.
- Enter accurate codes for services performed and bill time in patient care accurately.
- Provide consultation in a time‑efficient manner for patients, specific to care area; request orders and/or provide guidance to the patient’s team regarding activity or prevention of complications.
Provide Skilled Rehabilitation Treatment
- Perform and document skilled patient care activities, including direct treatment and caregiver/patient/family education.
- Monitor and adjust treatment plans specific to patient progress.
- Fabricate and adjust splints to meet patient needs.
- Communicate patient needs and promote carryover of strategies, precautions, etc. with nursing and other caregivers.
- Update recommendations to team regarding continuing rehabilitation needs and plan as patient’s condition and situations warrant.
- Direct, support and provide supervision as required for COTAs, aides and volunteers.
- Develop and enhance patient care program and patient outcomes in specialty area.
- Document treatment and other patient contacts in patient medical record in compliance with DNV, OHSU, OAR’s and third‑party payer criteria.
Clinical Education
- Provide evidence‑based clinical instruction experiences and supervision for OT students.
- Ensure student completion of weekly goals; identify and facilitate achievement of affiliation objectives.
- Provide regular feedback and complete evaluations on schedule.
- Participate in supervision and training for students from other disciplines.
Care Coordination
- Collaborate with patient, family and medical team in multidisciplinary patient rounds, ensuring a common level of understanding of patient’s rehabilitation needs and providing recommendations for rehab after discharge.
- Request patient/family conferences as appropriate and participate in conferences representing department, heightening awareness of and advocating for rehab needs of patient.
- Enhance patient safety both during and after hospitalization by assisting the team in problem‑solving, including vision, ADL and cognition impairments.
- Provide feedback and educate physicians, fellows, residents and other health care providers about rehabilitation (including medical necessity for skilled rehabilitation services and appropriate referrals and expectations for rehab outcome and intensity at various levels of care).
