With compassion and commitment, Clinical Social Workers embody the Legacy mission of making life better for others. They do this by evaluating the psychosocial and mental health issues of patients to ensure transition planning, crisis management, and planning/coordination of both internal and external services. They assess intervention strategies to address behavioral and addiction issues, as well as end-of-life care and informed decision making about therapeutic options.

Our Clinical Social Workers are advocates not just for our patients, but also for their surrogates, family members, caregivers and health care providers.

Legacy Emanuel Medical Center in North Portland plays a vital role as a local and regional leader in serious clinical illness or injury, with around-the-clock expertise for critical health issues.

Responsibilities
  • Provide social work services in a variety of adult and pediatric settings across the health care continuum with a focus on holistic patient-centered care and attention to the needs of vulnerable populations.
  • Work independently and as part of an interdisciplinary team.
  • Conduct comprehensive evaluation of psychosocial and mental/behavioral health status, legal and safety considerations, substance use or addiction, chronic illness, or end-of-life issues.
  • Provide brief counseling, transition planning, crisis management and planning/coordination with internal and community stakeholders.
  • Deliver social work intervention with individuals, family members or groups, using a strengths-based approach to enhance capacity to address needs.
Qualifications
  • Clinical Social Worker I CSWA Position Requirements:
    Education – Master of Social Work degree from an accredited School of Social Work required.
  • Experience – Preferred: 1 year of relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization.
  • Clinical Social Worker II LCSW Position Requirements: 2 years relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization. Preferred: 3 years' relevant experience.

Skills

  • Broad knowledge of social work principles and ability to apply them with individuals, families and/or groups, considering development, illness, and cultural/literacy factors.
  • Knowledge of medical, mental health and substance use diagnoses, disabilities, treatments, medications and terminology.
  • Strong organizational, communication and collaboration skills; ability to triage needs and function on an interdisciplinary team; ability to document clearly.
  • Knowledge of social determinants of health, diversity/equity/inclusion, and culturally responsive practice.
  • Ability to access and advocate for patient/family resources; understanding of legal/regulatory issues; proficiency with electronic medical records and basic computer programs.
  • Awareness of current health care environment and compliance requirements.
Licensure
  • Clinical Social Worker I CSWA – CSWA license (working toward LCSW) required in OR. CPR (AHA BLS) certification required.
  • Clinical Social Worker II LCSW – LCSW license required in OR. CPR (AHA BLS) certification required.
Pay Range

USD $39.27 - $56.16 / Hr.
