The Certified Recovery Mentor provides case management offering a combination of referral linkage, individual counseling, family support, group counseling and case management services to assist clients in successful engagement in treatment and families in supporting ongoing treatment involvement. May work in a residential or outpatient setting.

Location: Rockwood and Milwaukie, Oregon.

Pay: $21.74 - $28.26 hourly, plus $2.00 per hour additional salary for Bilingual Spanish proficiency.

Essential Responsibilities:

• Screens and selects clients to participate in the mentoring program.
• Develops the service plan for all clients, including a service reporting plan on each referral.
• Within the first five working days following intake, the client and (if appropriate) family members meet with the Recovery Mentor and Primary Counselor to develop a case management plan.
• Provides daily contact with clients for up to the first five weeks after release, helping with engagement in recovery support, housing and employment issues, family adjustment, legal, housing, medical and benefit issues. Frequency of contact gradually decreases to twice monthly or monthly by the 90th day after release.
• Assists the family in the initial implementation of the plan, maintains regular communication with primary therapist, collateral providers, community corrections staff, and links client to needed services.
• Determines when to terminate clients from the program because services are no longer necessary or the client is not responding to the mentor model.
• Documents and maintains client records in electronic healthcare system within 24 hours of meetings to ensure compliance with agency and contract guidelines and confidentiality.
• Meets regularly with staff from social service agencies to update them on services, identify unmet needs, review and update referral and case coordination procedures, and remove barriers to treatment.
• Some evenings/weekends required.
• Travels between sites or in the community to drive clients when needed.

Requirements:

• High school diploma or GED.
• Current Certified Recovery Mentor Certificate (CRM) with MHACBO.
• Substantial experience with addiction issues and understanding of the subculture.
• Ability to maintain confidentiality and appropriate boundaries.
• Extensive knowledge of navigating the social service network.
• Ability to accompany clients throughout their environment.
• Knowledge of the legal/corrections system.
• Effective communication skills and ability to maintain a cooperative working relationship in a culturally diverse environment.
• Basic computer skills (keyboarding, mouse, internet navigation).
• Willingness to work evenings and/or weekends.
• Valid driver's license, proof of current automobile insurance coverage, safe driving record verification, and ability to meet driving requirements.
