Our Hepatitis C Elimination Program (also known as the Hep C Team) provides low-barrier, trauma-informed care with a focus on harm reduction. We offer free or low-cost testing and treatment to most patients, ensuring you get the care you deserve. Our outreach teams collaborate with organizations such as CODA, Allied CTC, and FORA Health to provide testing in the community.
This position is housed in the Central City Concern (CCC) Pharmacy and will involve the coordination of services for CCC patients in the Hepatitis C (HCV) treatment program, working closely with providers, patients and pharmacy staff. Hep C Outreach and Linkage to Care Specialist scope will include a unique combination of administrative, research and patient care services. This role is especially relevant for individuals pursuing opportunities for direct patient contact, increased knowledge of pharmacy, meeting facilitation, program planning, research experience, and leadership opportunities.
The Hep C Outreach and Linkage to Care Specialist will work closely with partner programs within Central City Concern and in the community, thereby improving health outcomes and patient experience as part of a comprehensive approach to ending homelessness and achieving self-sufficiency.
Schedule: Monday - Friday, 8:00am - 4:30pm
Location: Old Town Clinic (727 W Burnside St. Portland, OR 97209)
Compensation: $24.52 - 31.88/hr
Minimum Qualifications- High school diploma or GED plus two years of experience working in community health, including at least one year working with homeless, low income, or other vulnerable populations in a supportive role, or working with populations living with HCV/HBV/HIV. A Bachelor’s degree in human/social services or closely related field can substitute for one year of the required community health experience.
- Must possess a current driver’s license, pass a DMV background check and be designated an acceptable driver as outlined in Central City Concern’s Fleet Safety Policy. Must pass an initial driver training and an annual recertification training.
- Strong cultural awareness of people who use drugs and how this may affect or influence service delivery.
- Bilingual speaker of Spanish preferred.
- S k i l led in Microsoft Office Suite, including Excel, preferred.
- Has the competencies necessary to deliver exemplary written and oral communications; understand healthcare-related terminology; and provide outreach, engagement and supportive services.
- Demonstrated ability to work independently with accountability and exercise sound judgement, discretion, and professionalism at all times.
- Must have current CPR certification prior to start.
- Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
- Must generally have the ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects and climb stairs, with reasonable or no accommodation.
