OT - Occupational Therapist

We are seeking a dedicated Home Health Occupational Therapist to provide skilled therapy services to patients in their homes. This position offers a 1:1 patient ratio with the possibility of visiting up to 6 patients per day, while maintaining high standards of productivity.

• Conduct assessments and develop personalized care plans for patients.
• Implement therapy programs focusing on rehabilitation and recovery.
• Educate patients and their families on treatment procedures and home exercise programs.
• Utilize OASIS for patient documentation and ensure proper record-keeping.
• Manage and optimize patient mobility and independence through therapeutic techniques.

• Minimum of 1 year of skilled home health experience required.
• Current BCLS certification is required.
• OASIS experience preferred; please indicate if not available.
• Must have reliable transportation for travel between patient locations.
