Summary

Responsible for providing care to clients in their homes which include light housekeeping, meal prep, assistance with daily living, medication management, memory care support, and help with many other special needs.

Essential Functions

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Assists clients with activities of daily living (ADLs), such as bathing, toileting, dressing and personal hygiene, ambulation, and household safety. Accompanies client to appointments and local trips.
  • Performs general housekeeping including cleaning, laundry, and meal preparation.
  • Redirects clients with memory issues; providing safety guidance protection to the client who cannot be left alone due to impaired judgment. Maintains excellent and objective ongoing communication with the SHC office staff; respond to phone calls and/or emails in a timely manner.
  • Administers medication with nurse supervision and track medication on a Medication Administration Record.
  • Provides companionship, including activities, discussions, encouragement, reassurance, and redirection as needed.
Required Skills/Abilities

Consistent execution of quality customer service. Maintains and demonstrates ethical conduct. Positive and constructive attitude. Must demonstrate ability to cooperate with others to contribute to the overall well‑being of the client in their home. Demonstrate ability to recognize a change in a client’s status, including physical, mental and emotional areas.

Working Environment & Physical Demands

May be exposed to confused, belligerent or physically aggressive clients, may be exposed to contagious diseases and mental stress. Manual dexterity, good vision and hearing required. May be exposed to bodily fluids.

Physical Demands Summary

This role requires frequent physical activity involving light, moderate, and heavy lifting and carrying, with repeated handling of loads up to 50 pounds throughout the day. Duties include pushing wheelchairs over varying distances, occasional overhead reaching, and periodic twisting, bending, turning, and squatting. The position also involves regular walking, standing, and sitting, reflecting a job that is physically demanding and requires sustained mobility and strength across the workday.

Required

Education, Experience, Licenses, and Certifications
  • High school diploma or equivalent required.
  • Verifiable experience and/or training in providing care for seniors or disabled persons.
