Position: Fit Case Manager #390

FIT Case Manager

Works with families involved with ODHS in Multnomah County who are also struggling with substance use disorder. Some focus on women and children in the African American culturally focused residential program Project Network and some focus on outpatient clients. Provides case management resources for clients and their families, as well as coordinating services.

Location: This position is based out of Life Works NW's King site.

Pay: $21.74 starting hourly pay working 16 hours per week.

Essential Responsibilities
  • Work closely with ODHS, FIT partners, PNet and AOP clinical teams, family members, and community partners to ensure a successful course of treatment at LWNW.
  • Screen clients and determine case management needs.
  • Accompany and represent clients in court.
  • Develop goals on Service Plan that facilitate each client's successful completion of their Individual Services and Supports Plan (ISSP).
  • Facilitate engagement in services. The FIT Case Manager is the primary source of communication between treatment services and ODHS, referents, family members, and other community partners.
  • Provide culturally-focused case management services to each client, as indicated in the Service Plan, including locating and accessing needed resources that are appropriate for the family, removing barriers to treatment success, facilitating movement between levels of care, assisting in the development of culturally-specific and recovery-oriented support systems, and skills training. Case management services are offered in multiple settings, including the community, residential treatment, and at home.
  • Maintain awareness of culturally appropriate community referral resources.
  • Provide home visits to offer parenting and child development information along with resource and referral information e.g., health, mental health services, community supports, etc.
  • Complete documentation in electronic health record system.
  • Serve as group facilitator; planning and implementing culturally appropriate group activities, including activities in the community that foster community awareness, educational success, development of social support systems, and recovery-oriented recreation.
  • Check e-mail/voicemail daily when working.
  • Some evenings/weekends required.
  • Travel between sites or in the community, including providing client transportation.
Qualifications
  • High school diploma/GED.
  • Must have a current Certified recovery Mentor Certificate (CRM) with MHACBO.
  • Demonstrated ability to communicate effectively; understand mental health/SUD assessment, treatment, and service terminology and individual, group, family, and other counseling techniques; understand program policies and procedures for services and supports identified in an Individual Services and Supports Plan.
  • Demonstrated ability to provide culturally-competent treatment services, understand cultural values and norms of diverse client populations, and maintain a cooperative working relationship with others in a culturally diverse environment. Sensitivity and understanding of specific barriers which may lead to lack of access and engagement.
  • Demonstrated ability in providing case management services and compliance with confidentially in accordance with federal, state and/or funding source requirements.
  • Knowledge of community resources and treatment agencies, enabling appropriate referrals for specific client needs.
  • Proficient in Microsoft Outlook and Word. Ability to master proprietary software including electronic health record system required.
  • Requires valid driver's license, proof of current automobile insurance coverage, verification of safe driving record from DMV, and ability to meet LWNW driving requirements.
