Program Description

We are seeking a Case Manager, QMHA to play a pivotal role in our Aid and Assist restoration program. This position is part of a two-person team supporting individuals under the Aid and Assist population. This role provides highly individualized and intensive recovery-oriented services to achieve specific goals and increase resiliency for individuals identified with a serious mental illness, which resulted in commission of a crime (Aid and Assist) as a result of their mental illness.

The Case Manager QMHA works with this program to support individuals transitioning from the Oregon State Hospital into the community. This position ensures continuity of care by initiating contact while clients are hospitalized, coordinating essential services such as housing and medication, and providing ongoing post-discharge support. The Case Manager serves as a bridge between institutional care and community integration, promoting stability, recovery, and independence.

Description

The Case Manager, QMHA is responsible for assisting clients access a variety of services and supports. This role serves as an advocate and works with clients to meet their goals and needs. This position values diversity and cultural differences within our Cascadia community and promotes integrated care, trauma-informed and person-first approaches, fostering an inclusive, safe, and accepting work environment.

Essential Responsibilities
  • Provide mental health case management and skills training with clients as indicated in their treatment plan and as directed by the treatment team.
  • Work with the treatment team on assessing treatment needs, supporting development and monitoring, providing consultation to mental health aides, and transition planning including referrals, screenings, advocacy, housing, resources, benefits, and client needs.
  • Provide individual and group skills training, CPST, activity therapy, and meet productivity expectations.
  • Engage clients to participate in treatment services.
  • Integrate best practices into day-to-day operations (Person-Centered Approach, Harm Reduction, Trauma-Informed Care, Motivational Interviewing, Strength-Based Approach, and Recovery Model).
  • Build cooperative relationships with outside providers to secure benefits, advocate for clients, and connect to acute care providers.
  • Coordinate treatment and continuity of care with community stakeholders, guardians, representatives from the legal system, hospital care, medical care, other community-based agencies, and Cascadia team members to maintain communication.
  • Attend and participate in clinical staff meetings to provide input and communicate program and client needs.
  • Provide monitoring and supervision to support clients in developing and maintaining structured activities aligned with personal goals and any required conditions for community integration.
  • Other duties as assigned.
Regulatory & Compliance
  • Comply with laws, regulations, policies, and procedures under which the program and company operate.
  • Assist in crisis situations following protocols and procedures in coordination with the treatment team.
  • Respect client rights and responsibilities, and demonstrate professional boundaries and ethics.
  • Maintain accurate and up-to-date documentation to ensure compliance with agency policies and local, state, and federal regulations and billing requirements.
