* Position Summary:

** The LVN provides direct nursing care to residents under the supervision of a Registered Nurse or Director of Nursing. This includes administering medications and treatments, supervising Certified Nursing Assistants (CNAs), monitoring patient care, and ensuring adherence to all regulatory and facility standards.###

* Key Responsibilities:

*** Supervise and support CNAs in delivering quality care
* Administer medications and treatments as prescribed
* Chart all care provided, resident responses, and any changes in condition
* Assist with admissions, discharges, and transfers
* Participate in care planning, quality assurance initiatives, and staff development
* Conduct rounds and ensure compliance with nursing procedures
* Respond to emergencies and provide critical nursing interventions
* Collaborate with interdisciplinary teams and families
* Maintain accurate, timely documentation in compliance with state and facility policies
* Promote resident rights, dignity, and confidentiality at all times###

* Qualifications:

*** Current and unencumbered LVN license in the state of California
* Graduate of an accredited LVN program
* CPR certification required
* Prior experience in long-term care or skilled nursing preferred
* Excellent communication, critical thinking, and leadership skills
* Proficient in basic computer use and electronic medical records (PCC experience preferred)###

* Physical Requirements:

*** Ability to lift 25+ lbs and perform frequent standing, walking, bending, and reaching
* Must be able to respond to emergency situations and provide hands-on care
* Capable of working under pressure in a fast-paced healthcare environment
