The Assisted Living Coordinator coordinates and addresses the health concerns of each Assisted Living Resident, including resident assessments, medication/pharmacy management and resident activities of daily living and personal care needs.

• Collaborate with the Director of Nursing (DON) and Assistant Director of Nursing (ADON) regarding day‑to‑day operation of clinical services for the entire campus.
• Ensure that each resident’s pre‑admission screen is completed prior to move‑in.
• Direct the Lifestyle and Care Needs Assessments of Residents to determine initial and ongoing medical and personal care requirements.
• Assist Residents with activities of daily living (ADL) including bathing, dressing, grooming, dining, etc., and medication administration.
• Lead and assist in developing theård plan in conjunction with the resident/family, ensuring communication of the plan to caregivers.
• Facilitate the Assisted Living Resident Meeting and follow up with feedback and concerns in a timely manner.
• Serve as the lead nurse for assisted living residents and participate in the on‑call rotation as designated by the DON.

• Active, unencumbered LPN license within the state.
• CPR certification.
• Previous assisted living and leadership experience preferred.
