• Wage: $28/hour

We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities.

• Only on call for triage of emergencies 1 week per month
• Mileage & Cell Phone Reimbursement
• Flexible Hours –5 day work week (40 hours, Monday thru Friday) NO Weekends

• Ensure that each individual receives quality, individualized health care management, resulting in their optimal health
• Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress
• Act as a health/medical resource for staff, training staff in the medical arena
• Conduct audits and reviews of medications
• Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records

• Full-time Monday-Friday with occasional travel required;
  flexible hours
• Participate in an on-call rotation for triage of medical concerns for 1 week per month

• Current Licensed Practical Nurse (LPN) license in the State of Indiana required
• 1-2 years experience working with individuals with disabilities or other long-term care is preferred
• Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
• Strong verbal/written communication skills
• Ability to work independently and efficiently
  
  Organizational and computer skills
• A successful background clearance is required as part of the onboarding/employment process
