RN Case Manager - Hospice

Precise Care Services is seeking an RN Case Manager for an 8-week Hospice assignment

Must have Home Health and/or Hospice experience.

Responsibilities:

As a Hospice Case Manager, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end-of-life care for the patient.

Qualifications:

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
