* * Home Health Social Worker responsibilities:**
* ** Assess
* * the psychosocial status of patients related to patient’s illness
* ** Develop and provide
** multiple community and governmental resources to support a patient in their home environment
* ** Provides
* * assessment in patient’s residence and carries out social evaluations and plan interventions
* ** Participate
* * in the development of the individualized plan of care
* ** Assist
* * physician in understanding significant social and emotional factors
* ** Participate
* * in quality assessment performance improvement team
* ** Serve
* * as liaison between patients and community agencies
* ** Maintain
* * and develop contracts with public and private agencies
*
* Qualifications:

**
* ** A graduate of an accredited master’s program in social work**
* ** Licensed MSW or LICSW in Washington State
*** Minimum of one year’s experience in health care
* Experience with home health or hospice preferred
* Possesses and maintains current BLS Certification
* A licensed driver with an insured automobile
